A child day program that is licensed and serves preschool-age children, shall be equipped with at least one carbon monoxide detector. There was not a carbon monoxide detector in the center.
Plan of Correction: Carbon monoxide detectors were immediately requested and and ordered through building services on 12/1/2025. Site Supervisor will ensure that a carbon

The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan. This list shall be dated and kept confidential in each room or area where children are present. Child #6 had listed in their record a dietary restrictions and it was not on the current list.
Plan of Correction: The Site Supervisor will ensure the child allergy/sensitivity/dietary restriction list is kept confidential in each room or area where children are present and is updated, signed and dated monthly with their routine audits.

Staff records for each person shall include all required items. Staff #2 and Staff #4 are missing documentation to demonstrate they possess the education required by the job position. Staff #2 and #4 are designated as program leaders.
Plan of Correction: The Program Supervisor has requested the required education documentation and will ensure moving forward that all education documentation is received during the application process, before hiring.
