An unannounced monitoring inspection was initiated on 9/17/24 and completed on 9/25/24 in response to a self-report received by the licensing office on 8/23/24 related to a reported injury of a child. The inspector reviewed 1 child's record and 2 staff records while on-site. This inspection included document review, interviews and observations of video footage. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.
The center did not maintain a written record of Child #1's injury. It was reported that an injury report was written on the date of injury, 8/22/24, however, the injury report was not available for review and was re-written on 9/24/24.

Plan of Correction: Corrected on site. The center will implement a duplicate copy system for all incident and accident reports to ensure a copy is placed and filed to maintained records.
