During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 3/25 /26. The on-site inspection began at 10:36am and ended at 1:43pm. The inspector reviewed compliance in the areas listed above. There were 114 children present and 28 staff. The inspector reviewed 8 children?s records and 8 staff records while on-site. This inspection included document review, tour of the facility and interviews.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/2/26. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.023-A
Notwithstanding any other provision of law, the Notice of Intent and related findings associated with the Superintendent's Special Order issued in April 2025 shall be posted in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations.

The Notice of Intent and related documents associated with the Special Order were not posted at the public entrance of the facility.

The medication authorization shall be available to staff during the entire time it is effective.

The medication authorization form on-file for an over-the-counter medicinal cream belonging to Child #19 was not renewed after 10 work days and the physician had not signed the authorization for it to be a long-term medication. The form was signed by the parent in September 2025.
Plan of Correction: The form shall be readily available to families. We now understand that hydrocortisone creams must have the 10 working day form.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include but not limited to the date, time and how the parent was notified of the child's injury and staff present at the time of injury.

Of 6 written injury reports reviewed, 1 report did not include this required information.
