Inspector Notes
An unannounced monitoring was conducted on June 6, 2023 from aproximately 2:30 pm-`3:40 pm. Children were observed watching a movie, singing songs,and freely playing. Staff records and children's records were reviewed . Licensing Inspector reviewed indoor and outdoor areas, supervision, activities, equipment,emergency evacuation and shelter-in-place drill documentation, emergency procedures and required posting.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
5Evidence: Staff #1 ( hire date 3/2/2023) resided in New York in the past five years. The center did not have documentation of submitting a Out of State Central Registry for the state of New York. Confirmed by Staff.
Evidence:
In the infant room, their was handsanitizers near the formula.
In the toddler room, their was handsanitizer on a table.
Evidence: The preschool playground composites had 2 to 3 inches of resilent surfacing. The playground composite should have 6 inches of resilent surfacing in the 6 feet fallzones.
Evidence:
Three of the four children in bucket seats, protective belts were not fastened securley.
Evidence:
There was a bottle in the infant room with out a name and date.
