Inspector Notes

An unannounced monitoring was conducted on June 6, 2023 from aproximately 2:30 pm-`3:40 pm. Children were observed watching a movie, singing songs,and freely playing. Staff records and children's records were reviewed . Licensing Inspector reviewed indoor and outdoor areas, supervision, activities, equipment,emergency evacuation and shelter-in-place drill documentation, emergency procedures and required posting.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, the licensee did not ensure obtain a copy of the results of a a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence: Staff #1 ( hire date 3/2/2023) resided in New York in the past five years. The center did not have documentation of submitting a Out of State Central Registry for the state of New York. Confirmed by Staff.

Based on observation, the licnesee did not ensure hazordous substances were placed in safe locking method that prevents access by children.

In the infant room, their was handsanitizers near the formula.
In the toddler room, their was handsanitizer on a table.

Based on observation, the licensee did not ensure fall zones had the required resilent surfacing.

Evidence: The preschool playground composites had 2 to 3 inches of resilent surfacing. The playground composite should have 6 inches of resilent surfacing in the 6 feet fallzones.

Based on observation, the licensee did not ensure when a child is placed in an infant seat or high chair, the protective belt shall be fastened securely.

Evidence:
Three of the four children in bucket seats, protective belts were not fastened securley.
