Inspector Notes

An unannounced monitoring inspection was conducted on11/21/2023 from 1:30 pm - 3:55 pm. At the time of entrance, 26 children, ages 8 weeks to 5 years of age, were in care with 5 staff members present. The sample size consisted of 5 children?s records and 5 staff records. Children were observed resting quietly on cots and in cribs, having an afternoon snack. Infants were being held, rocked, and bottle fed and were observed engaged in music and movement, crawling and toddling and playing with toys. Medication is not being administered at this time and children are not routinely transported in a vehicle. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of records, the licensee did not ensure that at least every two years from the date of the first initial screening or testing, or more frequently if recommended by a licensed physician or the local health department, staff members and individuals from independent contractors shall obtain and submit the results of a follow-up tuberculosis screening.
Evidence:
Five staff records were reviewed.
The record for staff #5 contained the most recent screening results for Tuberculosis (TB) which was dated 11/16/2021.
The Tuberculosis screening is more than 2 years old.
The program director confirmed that she did not have documentation of a more recent TB screening for staff #5.
Plan of Correction: The program director will speak with staff #5 today and staff #5 will be required to submit the results of an updated TB Screening within 2 weeks.

Based on a review of records, the licensee did not ensure that each staff member completes the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.

Evidence:
Five staff records were reviewed.
The record for staff #3, whose date of employment was documented in her record as 6/20/2023, did not contain documentation of the Virginia Department of Education-sponsored orientation course.
Staff #3 has been employed for more than 90 calendar days.
The program Director confirmed that she did not have documentation of staff #3 completing the Virginia Department of Education-sponsored orientation course.
Plan of Correction: The program director has already spoken to staff #3 regarding the need to complete the Virginia Department of Education-sponsored orientation course.
Staff #3 will complete the training and submit documentation of completion within 2 weeks.
