Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 8/13/2024. The on-site inspection began at 1:10 pm and ended at 3:40 pm. The inspector reviewed compliance in the areas listed above. There were 41 children present and 7 staff. The inspector reviewed 5
children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interview and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations

Five staff records were reviewed.
The records for staff # 1 (hired 9/18/2023), staff #3 (hired 1/22/2024), staff #4 (hired 4/9/2024) and staff #5 (hired 2/9/2024) did not contain results of a search of the central registry.
The program director confirmed that the results of the search of the Central Registry had not been received for staff #1, #3, #4 or #5.
Evidence:
Five staff records were reviewed.
The record for staff #3 (hired 1/22/2024) and staff #5 (hired 2/9/2024) did not contain documentation showing completion of the Virginia Department of Education-sponsored orientation course.
The program director confirmed that she was unable to demonstrate that this training had been completed by staff #3 and staff #5.

The records for staff #2 (hired 6/6/2022), staff #4 (hired 4/9/2024) and staff #5 (hired 2/9/2024) did not contain documentation to demonstrate that facility-specific orientation had been completed.
The program director was unable to provide documentation that staff #2, staff #4 or Staff #5 had completed a center specific orientation.
Evidence:
Two of the entry doors leading from the playground to classrooms are in disrepair to include large sections of peeling paint and splintering plywood.
Evidence:
There were 2 bottles of hand sanitizer located on top of a 4 foot shelf in the rear section of the 4 and 5 year old classrooms. Hand sanitizer has a high alcohol content and the label includes multiple warnings to include "Keep out of reach of children" and "call poison control... if ingested"
