Inspector Notes

A subsidy health and safety (SHSI) inspection was completed on8/13/2024 from 1:10 pm until 3:40 pm. At the time of entrance, 41 children were in care with 7 staff members present. The sample size consisted of 5 children?s records and 5 staff records

A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Standard 8VAC20-790-600-D-1
Based on record review and interview, the licensee did not ensure that all staff maintain current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care within 90 days of employment.
Evidence:
Five staff records were reviewed.
The records for staff #3 (hired 1/22/2024), staff #4 (hired 4/9/2024) and staff #5 (hired 2/9/2024) did not contain documentation to demonstrate certification in CPR.
The program director confirmed that staff #3, staff #4 and staff #5 were scheduled for an upcoming CPR and First Aid Training but had not yet obtained certification.

Based on record review and interview, the licensee did not ensure that all staff obtain first aid training within 90 days of employment.

Based on a review of records and interview, the licensee did not ensure that all staff who work directly with children shall, in addition to preservice and orientation training, annually attend at least 16 hours of training and staff development activities, to include the department's health and safety update course.
Evidence:
Five staff records were reviewed.
The record for staff #1 (hired 9/18/2023) and staff #2 (hired 6/6/2022) did not contain documentation of the department's health and safety update course.
The program director confirmed that staff #1 and staff #2 had not submitted a certificate of completion for the annual health and safety training.
