During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 7/28/2025 and completed on, 8/5/2025. The on-site inspection began at 1:40 pm and ended at 4:45 pm. The inspector reviewed compliance in the areas listed above. CDC: There were (#) children present and (#) staff. The inspector reviewed 5 children?s records and 6 staff records on-site on 7/28/2025. This inspection included document review, tour of the facility, interviews, observations and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 8/12/2025. A POC submitted after this date will not appear on the public website.

Staff #3's TB screening was completed more than 11 months prior to her date of employment.
Staff #1 did not have documentation of a Tuberculosis screening, which was more than 1 month overdue.
The posted menu listed Cheese stick, Cheezits and water for snack on the day of inspection. Children were served fruit snacks and water in some classrooms and a variety of many foods in another classroom. Neither cheese sticks, nor Cheezits were served in any of the classrooms on this date.
The posted menu was not adjusted to reflect the variety of foods served on this date.
Neither Fruit snacks (gummies) nor water fulfil a required nutritional food component.
Several children in the infant room, where all children were identified as being under 20 months old, were eating "fruit snacks" which were chewie gummy pieces which are difficult for young children to chew and pose a potential choke hazard..
Children in the Two's classroom were eating whole grapes which, due to the shape and size when not cut in half pose a potential to become lodged in a child's throat if not properly chewed.
A 9 month old infant was sitting in an infant bouncy seat drinking a bottle which was propped up to her mouth by a rolled blanket positioned on her chest.
