Inspector Notes
An unannounced monitoring inspection was conducted on January 3, 2023. There were 85 children present with 12 staff supervising. The physical plant, menu's, attendance logs, evacuation drill logs, first aid kit, flashlights and radios, children's records, bottles and cribs and the emergency preparedness procedures were reviewed. There were no medications on site. Areas of non-compliance were cited on the violation notice. The children were observed engaging in a variety of educational activities, including alphabet recognition, number recognition and singing and reading stories. In the 3 year old classroom, a wooden top has been placed over the toilet tank and the wood is splintering and the paint is peeling.

Child #2, #3 and #4's records did not contain the work phone number and/or place of employment of each parent who has custody.
Child #2's record did not contain documentation of the addresses of two designated people to call in an emergency.
Child #1's record did not contain documentation of the names of person authorized to pick up the child in an emergency, if the parent can not be reached.
