Inspector Notes

An unannounced renewal inspection was conducted today from 10:00am-11:45 am. There were 58children (infants-5yrs.) directly supervised by 11staff. The physical plant, staff records, children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and participating in circle time summer camp activities. Diapering and hand-washing procedures were also observed. There was an ample supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice.

Based on record review, required documentation of children's injuries were found to be incomplete.

Evidence: 8 of 11 accident injury reports were missing " Any future action to prevent recurrence of the injury" completed by program.
Plan of Correction: We will remind teachers to fill out information. It seems to be one classroom.
Standard 8VAC20-780-60-A
Based on a review of children's records, the center failed to obtain all required documentation for each child enrolled.

Evidence:
Child #1's record did not contain the work address of each parent who has custody and annual update.
Child #2's record did not contain the work address of each parent who has custody and annual file update.
Child #3's record did not contain annual file update/ review.
Plan of Correction: We will add lines for parents document.
