Inspector Notes
An unannounced monitoring inspection was initiated with an onsite inspection on 6/2/2025 with a local agency in response to a self-report received by the licensing office on 5/29/2025 relating to Staffing and Supervision. The inspector reviewed 1 child records and 2 staff records electronically on 6/9/2025. The investigation concluded on 9/22/25 completed. This inspection included document review tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/2/2025. A POC submitted after this date will not appear on the public website.
Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance. A three-year-old child was left unattended on the unsecured playground for approximately 11 minutes. The child was seen attempting to get the attention of persons inside the building including but not limited to knocking on the door, jumping up to be seen in the window and trying to reach the doorbell, crying, screaming, attempting to climb the fence and finally turning a bucket upside down climbing on top of it in an attempt to reach the doorbell.
Plan of Correction: HRCAP Head Start is dedicated to ensuring all students receive a quality education in a safe and healthy environment. In order to prevent child incidents, related to Active Supervision, the HRCAP Head Start Active Supervision policies and procedures have been reviewed and updated to reflect the need of the program. Additionally, all HRCAP Head Start staff will receive Active Supervision training twice a year. Each site will also receive weekly active supervision monitoring to ensure that all supervision policies and procedures are being followed.

Children under 10 years of age always shall be within actual sight and sound supervision of staff. A three-year-old child was not left alone and unsupervised outside on a playground for approximately 11 minutes. There were not any staff present on the playground during this time.

The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with the center's own policies and procedures that are required by these standards. Center staff did not follow the center policies supervision when coming into the building from the playground nor did they follow policy once back in their assigned classroom. As a result, a three-year-old child was left alone and unsupervised on the playground for approximately 11 minutes.
