Inspector Notes

A Code compliance monitoring inspection was initiated on 4/25/2024 and concluded on 4/25/2024. There were two classrooms with a total of 24 children present with four staff supervising. A total of five child records and six staff records were reviewed. The inspection started at 9:50 am and concluded at 11:50 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and date to be corrected? for each violation cited on the violation notice and return it to me within five (5) business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring gain; and 3) person or person(s) responsible for implementation.

Based on record reviews, the facility failed to ensure that all staff submit to a Fieldprint fingerprint background check prior to employment.
Evidence:
There are no fingerprint background checks on file for staff # 1, 2, 3, and 6.
Plan of Correction: The administration was made aware of violations. the school office team will secure missing documents. When document are secured we will notify DSS.

Based on record reviews, the facility failed to ensure to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence:
The sworn statement for staff #5 indicated the staff person had resided in another state in the preceding five years.
