Inspector Notes
An unannounced monitoring inspection was conducted today between the hours of 10:25 am-1:00 pm. There were a total of 115 children with 17 supervising staff members present. Children were observed during painting, puzzles, handwashing, bathroom breaks, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Violations
6Evidence:
1. Staff #1, Staff #2, and Staff #3's records were missing documentation of training in first aid and cardiopulmonary (CPR).
2. Staff #4 stated that Staff #1, Staff #2, and Staff #3 did receive complete orientation in first aid and cpr, but did not complete documentation for the training.
Evidence:
1. Staff #2's record (Date of hire-5/2/22) did not contain documentation of a completed orientation.
2. Staff #3's record (Date of hire-7/6/22) did not contain documentation of a completed orientation.
Evidence:
1. In the Art Studio classroom, there was peeling paint located in the sensory table area.
2. In the Nature Center's classroom, there was peeling paint located near the exit door and an edge of a large area rug was rolled up on the edge. This rolled edge created a tripping hazard.
3. On the toddler playground, there was standing water in the sandbox area. A tarp that was used to cover the sandbox collected two inched of water. Also, there was a tire being used to hold the tarp down on the sandbox that collected six inched of water inside of the tire.
Evidence:
In the Trailblazers classroom(2 year old classroom) a purse was opened and located on a low shelf.
Evidence:
1. A monthly practice evacuation drill was not conducted for the month of July.
2. Staff #4 confirmed that a practice evacuation drill was not conducted for the month of July.
Evidence:
1. Staff #2's record (Date of hire-05/02/2022) did not contain documentation of a second reference check and the fist reference check was completed after the date of hire.
