Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on October 9, 2024, as a part of the licensure period. The on-site inspection began at 9:30amand ended at 12:45pm. The inspector reviewed compliance in the areas listed above. There were 89 children present and 16 staff. The inspector reviewed seven children?s records and five staff records on-site.
This inspection included document review , tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 17, 2024. A POC submitted after this date will not appear on the public website.
Violations
9An annual fire inspection report from the appropriate fire official was not provided. Documentation of the last fire inspection report was dated 06/06/2023.
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.
In the Imagination A (4 year old)classroom, a box of floor wet cloths with the label "keep out of reach of children" was located in an unlocked low cabinet.
In the Imagination A (4 years old) classroom, a container of diaper ointment was not labeled with the child's name and made accessible to children.
Staff #5 was alone with a group of six two year old children. Staff #5's certification in CPR expired one month ago.

The center did not practice a monthly practice evacuation drill for the month of September. Staff #1 confirmed that the center did not conduct a practice evacuation drill for the month of September.
Child #6 and Child #7, who have been enrolled at the center for one and a half months, have a diagnosed food allergies and did not have a written care plan in their file.
Staff records shall be kept for each staff person.
Documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position was not kept for Staff #1, Staff #2, and Staff #3.
Staff #1, who has been actively working at the center for the past five months, did not have two or more completed reference verification on file.
Child #3 and Child #4's record did not contain signed documentation of authorization for emergency medical care, notification of a communicable disease, and pick up of ill child.
