Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on March 10, 2025, as a part of the licensure period. The on-site inspection began at 9:50am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were 87 children present and 19 staff. The inspector reviewed five children?s records and six staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Repeat Violation

he center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction.

The center did not have documentation of an annual fire inspection at the time of the inspection. Documentation of the last annual fire inspection was dated February 1, 2023.
Plan of Correction: Management will immediately call the local fire marshal and schedule a fire inspection. Moving forward, management will ensure to schedule annual fire inspections for the center and maintain documentation of the inspection report.

Repeat Violation

At least one staff in each classroom or area where children are present shall have current certification in both CPR and first aid.

Staff #1 and Staff #6 were working together with a group of six three year old children in Classroom H and were not certified in CPR or first aid.
Plan of Correction: Staff that are certified in first aid and cpr will be utilized in the classroom until Staff ! and Staff #6 are certified in first aid and cpr. Moving forward, management will ensure that at least one staff member in a group of children are first aid and cpr certified trained.

Repeat Violation

Documentation of at least 2 references to character, reputation, and competency are to checked prior to employment.

Staff #1, Staff #4, and Staff #5, all who have been actively working at the center for over one month, did not have any completed references verifications on file.
Plan of Correction: Moving forward, management will utilize the department's staff audit document to ensure that all staff files are complete with all required items. Two reference checks will be conducted for all newly hired staff before employment and documented in the staff member's record.
