Inspector Notes

The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on October 26, 2023 from approximately 10:00 AM to 11:30 PM. There were 37 children in attendance and a total of seven staff supervising. A total of five children and five staff records were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-560-A
Based on review of five staff records, the vendor did not ensure one staff was evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB) within the required timeframe.
Evidence:
1. The record for Staff #4 (Date of Employment 04/06/2023) contained a TB screening dated 04/13/2023.
2. The documentation of the TB screening is required to be obtained within the last 30 calendar days of the date of employment and be signed by a physician, physician?s designee, or an official of the local health department.
Plan of Correction: Going forward, we will make sure all newly hired staff obtain the TB screening within the required time frame.
Standard 8VAC20-790-640-A
Based on observation, the vendor did not ensure all hazardous substances such as cleaning materials, insecticides, and pesticides were kept in a locked place using a safe locking method that prevented access by children.
Evidence:
1. Several containers of cleaning chemicals were in a cabinet in the Twos & Threes classroom.
2. The lock on the cabinet was broken.
Plan of Correction: The cleaning chemicals were moved to a location that locked while the inspector was on the premises.
