Inspector Notes

This annual subsidy inspection was conducted by the inspector using an alternate remote protocol necessary due to a state of emergency declared by the Governor of Virginia in response to the COVID-19 pandemic.
Preliminary data was obtained from the vendor on 3/25/2021.
An annual subsidy inspection was conducted on 3/30/2021, at which time the vendor was contacted by video call to conduct the inspection. The inspector emailed the vendor a list of items required to complete the inspection.
The vendor reported there were 17 children in care with three staff. The inspector reviewed three staff records submitted by the vendor to ensure compliance with the health and safety regulations.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for
Child Care programs.
Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

Based on review of three staff records, it was determined that the vendor did not ensure that all staff completed a sworn statement or affirmation prior to their first day of employment.

Evidence:

1. Staff #1's date of hire was 3/9/21 and the sworn statement or affirmation was signed on 3/10/21.
2. Staff #3's date of hire was 12/17/20 and the sworn statement or affirmation was signed on 1/11/21.
Plan of Correction: All new staff will sign sworn statement or affirmation prior to their first day of employment.
