Inspector Notes

An unannounced, on-site code compliance inspection was initiated on February 24, 2025 and completed on March 4, 2025. The on-site inspection began at 12:40pm and ended at 2:18pm. The inspector reviewed compliance in the areas listed above. There were 32 children present with 4 staff. The inspector reviewed 5 children?s records and 7 staff records on-site. This inspection included document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance(s) with applicable code sections, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/18/25. A POC submitted after this date will not appear on the public website.

All areas on the premises accessible to children are free from obvious injury hazards. The following hazards were observed in the outdoor play area.
1. A basketball hoop approximately 10 feet tall was leaning over the wooden fence.
2. Multiple areas of the wooden fence have loose or missing pieces.
3. The seesaw and merry go round are rusted.
4. The following hazards are a potential poking or snagging injury: Multiple areas of the wooden fence have sharp protruding nails, the plane shaped climbing structure has a broken pipe on the wing and the roof of the plastic playhouse has exposed sharp pieces and two sharp protruding nails.
Plan of Correction: -Basketball hoop has been adjusted and placed upright.
-Fence repaired and photos emailed to inspector.
-Seesaw and Merry-go-round will be painted on 3/19/2025.
-Nails on fences have been nailed down.
-Playhouse was painted and photos emailed to inspector.
