A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

The child's record shall include immunization records for the child received on or before the child's first day of attendance and written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.
The record for child #1 did not have documentation of immunization records. The child was in care during the inspection and has been in care for at least two years.
The record for child #1, child #2, child #3, child #4 and child #5 did not have documentation of a written authorization for emergency medical care.

The child's record shall include proof of a child's identity and age. The record for child #2 did not have documentation of proof of identity.

Standard 8VAC20-790-640-A
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. Two bathrooms had a spray can of air freshener on the safety grab bar and a cabinet had various cleaning materials all with the warning label ?keep out of reach of children? The hazardous substances were unlocked and within reach of children.
Plan of Correction: Cleaning bottle was removed immediately and placed in staff cabinet with lock.

There shall be at least 12 inches of space between occupied cribs, cots, beds, and rest mats. During nap time, 12 occupied cots had less than 12 inches of space in between them.
Plan of Correction: Staff advised to maintain 12 inches between all cots. Corrected immediately.
