Based on review of five children's records, the vendor did not ensure the records of three children?s records contained the required information.
Evidence:
1. The records for Child #1, Child #2, and Child #3 did not contain the address for the listed emergency contact.
2. A member of management confirmed this information while the licensing inspector was onsite.

Based on review of six staff records and interview, the vendor did not ensure four staff records contained the required information.
Evidence:
1. The records for Staff #2 (Date of Employment 06/10/2020), Staff #3 (Date of Employment 05/30/2023), Staff #4 (Date of Employment 04/28/2023), and Staff #5 (Date of Employment 06/12/2023) did not contain documentation of orientation.
2. A member of management stated orientation was completed with the staff.
3. The documentation of orientation could not be located during the inspection.

Based on review of six staff records, observation and interview, the vendor failed to obtain satisfactory results of the Virginia Child Protective Services Central Registry check for one staff member.
Evidence:
1. The record for Staff #2 (Date of Employment 06/10/2020) did not contain results of the Virginia Child Protective Services Central Registry check.
2. The staff were observed working with children in the center during the inspection.
3. A member of management confirmed this information during the inspection.

Based on review of six staff records, the vendor did not ensure three staff were evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB) within the required timeframe.
Evidence:
1. The record for Staff #4 (Date of Employment 04/28/2023) contained a TB screening dated 01/18/2023.
2.. The records of Staff #5 (Date of Employment 06/12/2023), and Staff #6 (Date of Employment 09/25/2023) did not contain documentation of a TB screening.
3. Staff #4, Staff #5, and Staff #6 were observed working in the center.
4. The documentation of the TB screening is required to be obtained within the last 30 calendar days of the date of employment and be signed by a physician, physician?s designee, or an official of the local health department.

Based on the review of five children's records, the vendor did not obtain documentation that each child had received the immunizations required by the State Board of Health before the child attended the center.
Evidence:
1.The records for Child #1 (first day of attendance 05/15/2023), Child #2 (first day of attendance 05/15/2023) and Child #5 (first day of attendance 08/23/2021) did not have documentation of immunizations.
2. Child #1, Child #2 and Child #5 were observed in care during the inspection.
3. A member of management confirmed this information while the inspector was on site.

Based on review of six staff records and interview, the vendor did not ensure within the first 90 days of employment one staff who worked directly with children completed Virginia Preservice Training for Child Care Staff.
Evidence:
1.The record for Staff #5 (Date of Employment 06/12/2023) did not contain documentation of completing Virginia Preservice Training for Child Care Staff.
2. Staff #5 was observed working with children during the inspection.
3. A member of management confirmed this information during the inspection.

Based on review of six staff files and interview, the vendor did not ensure orientation training for one staff was completed prior to the staff member working alone with children and within seven days of the date of employment.
Evidence:
1. The record for Staff #6 (Date of Employment 09/25/023) did not contain documentation of orientation that was partially completed.
2. A member of management stated orientation was initiated with Staff #6 but was not finished due to an emergency.

Based on review of six staff records, interview and observation, the vendor did not ensure all staff who work directly with children obtained current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care within 90 days of the date of employment.
1. The records for Staff #3 (Date of Employment 05/30/2023), Staff #4 (Date of Employment 04/28/2023), and Staff #5 (Date of Employment 06/12/2023) did not contain documentation of current CPR certification.
2. Staff #3, Staff #4 and Staff #5 were observed working with children during the inspection.
3. A member of management stated a training needed to be scheduled for the staff to receive the required training.

Based on review of six staff records, interview and observation, the vendor did not ensure all staff who work directly with children obtained current certification in first aid appropriate to the ages of children in care within 90 days of the date of employment.
1. The records for Staff #3 (Date of Employment 05/30/2023), Staff #4 (Date of Employment 04/28/2023), and Staff #5 (Date of Employment 06/12/2023) did not contain documentation of current first aid certification.
2. Staff #3, Staff #4 and Staff #5 were observed working with children during the inspection.
3. A member of management stated a training needed to be scheduled for the staff to receive the required training.

Based on observation and documentation review, the vendor failed to ensure pillows were not used by children less than 12 months of age while sleeping or resting.
Evidence:
Infant #1 (six months of age) was observed sleeping in the crib. A boppy pillow was placed in the crib, underneath the head of Infant #1.

Based on documentation review and interview, the vendor did not maintain a record of the dates of the practice drills for one year.
Evidence:
A member of management stated the required emergency drills had been completed but not documented on the drill logs.

Based on observation and documentation review and interview, the vendor failed to ensure bottles were not used while the child is in his designated sleeping location.
Evidence:
Infant #1 (six months of age) was observed lying in the crib, drinking a bottle.
