Inspector Notes
The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on October 26, 2023 from approximately 11:30 AM to 1:30 PM. There were 26 children in attendance and a total of six staff supervising. A total of five children and six staff records were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice, sign and returned it to me within five business days from today. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

1. The records for Staff #2 (Date of Employment 06/10/2020), Staff #3 (Date of Employment 05/30/2023), Staff #4 (Date of Employment 04/28/2023), and Staff #5 (Date of Employment 06/12/2023) did not contain documentation of orientation.
2. A member of management stated orientation was completed with the staff.
3. The documentation of orientation could not be located during the inspection.

1. The record for Staff #2 (Date of Employment 06/10/2020) did not contain results of the Virginia Child Protective Services Central Registry check.
2. The staff were observed working with children in the center during the inspection.
3. A member of management confirmed this information during the inspection.
Evidence:
1. The record for Staff #4 (Date of Employment 04/28/2023) contained a TB screening dated 01/18/2023.
2.. The records of Staff #5 (Date of Employment 06/12/2023), and Staff #6 (Date of Employment 09/25/2023) did not contain documentation of a TB screening.
3. Staff #4, Staff #5, and Staff #6 were observed working in the center.
4. The documentation of the TB screening is required to be obtained within the last 30 calendar days of the date of employment and be signed by a physician, physician?s designee, or an official of the local health department.

1.The records for Child #1 (first day of attendance 05/15/2023), Child #2 (first day of attendance 05/15/2023) and Child #5 (first day of attendance 08/23/2021) did not have documentation of immunizations.

1.The record for Staff #5 (Date of Employment 06/12/2023) did not contain documentation of completing Virginia Preservice Training for Child Care Staff.

2. A member of management stated orientation was initiated with Staff #6 but was not finished due to an emergency.

Infant #1 (six months of age) was observed sleeping in the crib. A boppy pillow was placed in the crib, underneath the head of Infant #1.
Evidence:
A member of management stated the required emergency drills had been completed but not documented on the drill logs.
Evidence:
Infant #1 (six months of age) was observed lying in the crib, drinking a bottle.
