A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Standard 8VAC20-790-560-A
Staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Staff #3 TB was dated four months prior to date of employment and has been employed for over one year. Staff #4 is currently working and has a TB screening that was dated two weeks after their first day of employment.

Subsequent TB screenings are required at least every two years from the date of the initial screening, or more frequently if recommended by a physician.

Staff #5 updated screening was due 14 months ago. The updated screening has not yet been completed.
