Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/24/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-790-560-B
Repeat Violation:
Subsequent TB screenings are required at least every two years from the date of the initial screening, or more frequently if recommended by a physician.

Staff #5 had a TB screen that expired over 2 years ago.
Plan of Correction: Informed staff member that TB screening needed to be completed immediately.
Standard 8VAC20-790-600-F
Staff who work directly with children shall, in addition to preservice and orientation training, annually attend at least 16 hours of training and staff development activities.

Staff #4, employed for one year and eight months, had seven of 16 hours of training documented for 2024.
Plan of Correction: I reminder the staff member that they are required to complete 16 hours of personal development training annually.

The vendor shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the date and time when parents were notified.

Out of four injury reports reviewed, four were missing the date and time parents were notified.
Plan of Correction: Revise current injury report to include the date and time for the parent to fill out when signing incident reports.
Standard 8VAC20-790-730-F
Cribs, cots, rest mats, or beds shall be provided for children during the designated rest periods and shall not be occupied by more than one child at a time.

In the toddler classroom, a child was observed sleeping on the floor and not on a cot or rest mat as required.
Plan of Correction: Informed toddler/PreK staff members that all children must be on cots during nap time.
