63.2 Child Abuse & Neglect

Inspector Notes

A complaint investigation was conducted on July 1, 2024 from approximately 1:00pm until approximately 4:20pm regarding child abuse and neglect. The investigation was completed with two licensing inspectors and two other local agencies. A separate investigation was completed at the same time. Interviews were conducted and documentation was reviewed. There was not a preponderance of evidence to support the allegation regarding child abuse and neglect. Standard 8VAC20-770-60-C-2
Based on record review, the center did not deny continued employment for an employee that did not have a Central Registry finding within 30 days of employment.

Evidence:
The file for Staff #4 (DOH 4/12/2023), had completed Central Registry Results for a person other than the employee. The results in the file were for a person not employed at the center, with a different first name, date of birth, and social security number.
Plan of Correction: The last day of employment for Staff #4 was 6/21/2024.

Moving forward, the director will check that the names match on all required paperwork and background checks.
Standard 8VAC20-780-530-A
Based on record review, the center did not ensure that at least one staff in each classroom or area where children are present had current certification in cardiopulmonary resuscitation (CPR) and current certification in first aid.

Evidence:
Staff #1 and Staff #4, who were supervising a classroom, did not hold a current CPR and First Aid certification.
Plan of Correction: Moving forward, the center will ensure at least one staff will be CPR/First Aid certified when in a classroom or area with children.
Standard 8VAC20-780-550-G
Based on record review and interview, the center did not ensure documentation was maintained of emergency evacuation drills.

Evidence:
The Director acknowledged they did not document drills completed in the month of June.
Plan of Correction: The center will ensure all drills are documented properly before the end of the month.
