Inspector Notes

An unannounced inspection was initiated and completed on November 14, 2024 in response to a complaint received by the licensing office on November 12, 2024 relating to allegations of supervision and Special Care Provisions & Emergencies. The Department issued a Notice of Intent to Place Licensee on Probation and Require Licensee to Contact Parents, Guardians, or other Responsible Persons Regarding Health and Safety Violations on November 13, 2024 as a result of high-risk violations. The inspector reviewed 2 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on December 3, 2024. A POC submitted after this date will not appear on the public website.

The center is required to have employees undergo a background check every five years.

Standard 8VAC20-780-340-F
Children under 10 years of age shall always be within actual sight and sound supervision of staff.

On November 4, 2024 at approximately 5:30pm, Infant #1 (age 13 months) and Infant #2 (age 14 months) were left in the Infant classroom unsupervised for approximately two minutes while Staff #1 went to retrieve a bottle from a bag in the hallway.

The required ratio for children ages birth to 16 months is one staff per four children.

During the inspection, Staff #5 was observed leaving the Infant classroom to take out the trash leaving two staff supervising 10 infants. Later, Staff #5 was observed leaving the Infant classroom to put bottles away in the hallway.
During the inspection, Staff #4 from another Infant classroom left the classroom to put things away in the hallway, leaving that classroom with two teachers supervising 9 children.

The center shall notify the parent by the end of the day of any known minor injuries.

On November 12, 2024 at approximately 12:15pm, Staff #1 observed Infant #1 (age 13 months), bumping their face on a shelf. The parents were not notified by the end of the day of this incident. The parents noticed the injury after the infant was picked up and the injury required outside medical attention.
