During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on March 20, 2025, as a part of the licensure period. The Department issued a Special Order to place licensee on probation and required licensee to contact parents, guardians, or other responsible persons regarding health and safety violations on December 12, 2024 as a result of high-risk violations. The on-site inspection began at 10:15 a.m. and ended at 12:00 p.m. The inspector reviewed compliance in the areas listed above. There were 71 children present and 23 staff. The inspector reviewed 7 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on March 28, 2025. A POC submitted after this date will not appear on the public website.

The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.

Child #3, 13 months old, last documented immunization record was over 9 months ago and overdue by over 3 months. Child #5, 14 months old, last documented immunization record was over 7 months ago and overdue by over 1 month.
Plan of Correction: The parents were notified and updated immunization records will be obtained.
