Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on March 20, 2025 in response to a self-report received by the licensing office on March 19, 2025 relating to special care provisions and emergencies. The Department issued a Special Order to place licensee on probation and required licensee to contact parents, guardians, or other responsible persons regarding health and safety violations on December 12, 2024 as a result of high-risk violations. The on-site inspection began at 10:15 a.m. and ended at 12:00 p.m. The inspector reviewed 7 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
