Inspector Notes
An unannounced monitoring inspection was conducted today between the hours of 9:30 am through 10:45 am. There were a total of seven children (16 points) observed with three caregivers. A total of six children's files and six staff/household member's files were reviewed. The children were engaged in the following: toy play, circle time, bathroom breaks, hand washing, and other organized activities. Areas of non-compliance are identified in the violation notice. 1. Caregiver #1's record contained an expired fingerprint results report. Caregiver #1's record contained a fingerprint results reports dated 12/02/2020. Caregiver #1 confirmed that a new fingerprint search had not been conducted at the time of the inspection.
Evidence:
1. Household Member #1's record contained an expired fingerprint results report. The report was dated 04/02/2018. Caregiver #1 confirmed that a new criminal history check had not been conducted at the time of the inspection.

Child #2's record (Start date-02/03/2023) did not contain documentation of a physical examination.

1. Caregiver #1's record contained a TB screening that was expired. The TB screening was dated 12/02/2020. Caregiver #1 confirmed that a subsequent TB screening had not been completed at the time of the inspection.
2. Household Member #1's record contained a TB screening that was expired. The TB screening was dated 12/02/2020. Caregiver #1 confirmed that Household Member's subsequent TB screening had not been completed at the time of the inspection.

1. Child #1's record (Start date-02/01/2023) did not contain the name, address, and phone number for a second emergency contact.

1. Four of four containers of diaper cream did not have written parent authorization for Child #3, #4, #5, and #6.
2. Caregiver #1 confirmed that Child #3, #4, #5, and #6 did not have written parent authorization for the diaper creams.
Evidence:
1. Documentation of emergency evacuation drills were not maintained for the months of January, February, and March.
2. Caregiver #1 confirmed that the evacuation drills for the months of January, February, and March were completed, but the documentation was not completed.

1. Three bottles of breast milk were located in the refrigerator without a child's name or current date.
