Licensing Inspector Anderson conducted an unannounced complaint inspection in response to allegations received by the licensing office on August 16, 2022, in the areas of supervision & care of children. The investigation of the complaint was initiated on August 16, 2022 and findings were reviewed on October 12, 2022.
As the information gathered during the investigation support the allegations, the complaint is determined to be ?VALID.?

Based on record review & interviews, the religiously exempt center failed to implement established supervision procedures to ensure the safety of the children in care.
EVIDENCE:
Staff Member #1 did not implement the center's established procedures for appropriate oversight and supervision of children. Staff Member #1 allowed Child #1 (4 years old) to leave the classroom unsupervised, to go to the bathroom. The bathroom is located approximately 100 feet from the classroom and approximately 25 feet from the Center's exit door. Staff Member #1 did not have sight nor sound supervision. Child #1 left the day care using the exit door closest to the bathroom. The door was unlocked and equipped with a break-away bar.
EVIDENCE:

The bathroom is located approximately 100 feet from the classroom and approximately 25 feet from the Center's exit door. Staff Member #1 did not have sight nor sound supervision. Child #1 left the day care using the exit door closest to the bathroom. The door was unlocked and equipped with a break-away bar.
Plan of Correction: Director held a full training session with all staff on active supervision. All doors are kept locked. And ADT alarm was immediately installed

Based on interviews, the religiously exempt with subsidy center did not maintain actual sight and supervision for children less than 10 years of age nor did they maintain supervision by hearing a child who was using the restroom.
EVIDENCE:
Staff Member #1 allowed Child #1 (4 years old) to leave the classroom unsupervised to go to the bathroom. The bathroom is located approximately 100 feet from the classroom. Staff Member #1 did not maintain sight and sound supervision. Child #1 left the daycare and walked outside into the church parking lot, continued to walk down the street (Hill Street - a two-way busy corridor) approximately .60 miles, where the child was picked up by an unknown citizen who was driving by and noticed the young child walking alone. This citizen drove Child #1 to the nearest licensed day care center located on Hill Street. Community Person #4 (a current employee of the licensed day care center) contacted the child's mother through Facebook.
