Inspector Notes
An unannounced monitoring inspection was conducted on 10/17/2022 from 9:42am to 11:27am. At the time of entrance, 43 children were in care with 6 staff members present. Children were observed playing with table top toys, eating snack, working on Montessori activities, playing on the floor and listening to music. Interactions between the children and staff were positive. The site was organized and contained an abundant supply of developmentally appropriate materials. A selection of staff and children records, medications, the physical space, evacuation drills, emergency supplies and attendance records were reviewed. Standard 8VAC20-780-160-A
Based on review of staff records, the center did not obtain from each staff member a tuberculosis screening at the time of employment and prior to coming into contact with children that had been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
Evidence:
1. Staff #1 (date of hire 7/05/2022) had a tuberculosis screening dated 9/21/2021.
Plan of Correction: I will have it updated and in the future will make sure it is completed in the 30 days.
Standard 8VAC20-780-280-B
Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the 3year to 6 year old classroom, a bottle of Lysol spray and Febreze was inside an unlocked high cabinet.
Plan of Correction: The cleaning supplies were moved to the locked closet.

Based on review, the center had an injury prevention plan that had not been updated at least annually based on documentation of injuries and review of the activities and services.
Evidence:
1. The center's injury prevention plan was last reviewed on 9/02/2021.
Plan of Correction: I will update it today.

Based on review of children's records, the center did not obtain parent confirmation that the required information in the child's record is up to date at least annually .
Evidence:
1. Child B's record (start date 12/06/2019) did not have parent confirmation that the required information in the child's record was up to date at least annually.
Plan of Correction: I will ask for her to review and update.

Based on observation, the center did not treat children's records confidentially.
Evidence:
1. In the 3 year old/4 year old classroom, the center had an allergy list posted, but it did not have a cover page to keep the information confidential.
Plan of Correction: She covered it immediately.
Standard 8VAC20-780-60-A-8
Based on review of allergies, the center did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
1. Child B had a diagnosed food allergy and the center did not have a written care plan at the center.
Plan of Correction: I will ask for the action plan.
