Inspector Notes

An unannounced monitoring inspection was conducted on 5/3/2023 between the hours of 1:12pm-3pm. There were 79 children directly supervised by 14 staff. The physical plant, 3 staff records, 5 children?s records, medications, evacuation drills, injury reports, and emergency supplies were inspected. Children were observed during naptime, eating snack, and completing puzzles. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Based on record review and provider interview, the center did not meet the proof of child identity and age requirements as stated in ? 22.1-289.049 of the Code of Virginia.

Evidence:
1. Child #1 began attending the Child Day Center on 3/20/2023. Documentation in the record for Child #1 indicated the child's proof of identity and age was verified on 4/11/2023, more than seven days following the first date of attendance.
2. There was no documentation in the record for Child #1 of a report to local law enforcement when proof of age and identity was not verified within seven business days of initial attendance.
Plan of Correction: Moving forward, we will make sure we view the proof of age and identity within the seven days.
