Inspector Notes

An unannounced inspection was initiated on July 25, 2024 and completed on July 25, 2024 in response to a complaint received by the licensing office on July 22, 2024 relating to allegations of staffing, supervision, and programs. This inspection included document review, tour of the facility, interviews, and observations

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Though not a part of the original complaint, violations that were not reported were found related to terms of the license and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 10, 2024. A POC submitted after this date will not appear on the public website.

Based on observation, review of the license and interview, the licensee failed to ensure that the center?s activities are maintained in compliance with the terms of the current license issued by the department.

Evidence:
1. The center license is issued to 4501 Williamsburg Road, Suite L., Henrico, Virginia 23231. Children and staff were using another suite location in the same strip mall that was not licensed with the Virginia Department of Education.
2. Staff #1 and staff #2 stated that the staff and children use the suite located a few doors down from the licensed location for projects and activities.

Based on observation the center did not ensure that portable wading pools were not used for children that are not toilet trained.

Administration stated that children who are not potty trained used the portable wading pool at the center.
