Inspector Notes
An unannounced inspection was initiated on September 12, 2024 and completed on September 12, 2024 in response to a complaint received by the licensing office on August 21, 2024 relating to allegations of staffing and supervision. This inspection included, document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on December 2, 2024. A POC submitted after this date will not appear on the public website.

2Evidence:
On September 12, 2024, the following was observed in room #1. Times are approximate.
1. At 6:50 AM, a grouping of nine children were being supervised by one staff. The youngest in the group was two years old requiring a ratio of one staff to eight children.
2. At 6:55 AM a grouping of 10 children were being supervised by one staff. The youngest in the group was two years old requiring a ratio of one staff to eight children.
3. At 7:00 AM a grouping of 15 children were being supervised by one staff. The youngest in the group was two years old requiring a ratio of one staff to eight children.
4. At 7:05 AM a grouping of 16 children were being supervised by one staff. The youngest in the group was two years old requiring a ratio of one staff to eight children.
Evidence:
1. The centers policies and procedures state that ?State ratios are important indicators of quality. Ratio adherence is a dual responsibility between teachers and management.? ?Call the front desk for assistance? when ?the total number of children in attendance exceeds the state ratio?.
2. On September 12, 2024, in room #1, staff did not maintain staff to children ratios from 6:50 AM to 7:09 AM.
