During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on October 2, 2024 and completed on October 23, 2024 with a local agency in response to a complaint received by the licensing office on October 2, 2024 relating to allegations of supervision and special services. The inspector reviewed one child?s record and one staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on December 6, 2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

Child #1 was being transported from school to the childcare center. Staff #1 stated she did not see child #1 get up during transportation and move to a seat in the back of the vehicle. Staff #1 stated she asked another child if all the children were off the vehicle. Child #1 was left unsupervised in the back of the vehicle for less than 5 minutes.
Plan of Correction: Face to name recognition has been implemented and a log in roll for the children created for teacher receiving students. Management performs random day checks to ensure the procedures to keep children safe and accounted for is being followed. Drivers are required to put the vehicle in park, get out and check the children into the building with the receiving teacher.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.

Child #1 fell asleep in the back of the center vehicle and was unsupervised for less than five minutes.
Plan of Correction: Face to name recognition has been implemented and a log in roll for the children created for teacher receiving students. Staff shall verify that all children have been removed from the vehicle at the conclusion of any trip.

Staff #1 did not verify that all children had been removed from the vehicle. Child #1 was left unsupervised on the vehicle for less than five minutes.
