During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated on April 17, 2025 and completed on May 5, 2025 in response to a complaint received by the licensing office on April 16, 2025 relating to allegations of supervision, building maintenance, and nutrition and food service. The inspector reviewed four staff files on May 5, 2025. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on June 30, 2025. A POC submitted after this date will not appear on the public website.
Violations
4The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.
1. The floor in the infant room had a sticky residue, the rug had visible stains and dirt residue. Staff stated the rugs do not get vacuumed.
2. An outlet cover in the infant room has a gap from the wall to the outlet cover. An outlet outside of the school age room has come away from the wall and has tape covering the surface of the outlet cover and wall. Chipped and peeling paint was observed on the walls.
3. The floor in the pre-k room had a sticky residue, fruit was on the floor surrounding the trash can, children were on the playground at this time. Staff stated that the rugs do not get vacuumed.
4. In the preschool room, one of two doors leading into the room has a missing doorknob. The baseboard was missing in sections of the room.
When staff are supervising children, they shall always ensure their care, protection, and guidance.
During a transition from room #4 to the playground, Staff B and staff C were not aware that child #1 was left alone in the classroom for less than 5 minutes.
Children under 10 years of age shall always be within actual sight and sound supervision of staff.
Staff #A, staff B, and staff C stated that child #1 was left unsupervised in room #4 for less than 5 minutes during the transition to the playground.
The operational responsibilities of the licensee shall include ensuring that the center's services are maintained in compliance with the center's own policies and procedures that are required by these standards.
1. The center's supervision policy states staff should always be within sight and sound of all children at all times.
2. The center was in violation of their policy when a was left unsupervised in room #4. The child was not in sight or sound of a staff member for less than 5 minutes.
