The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.
1. In room #2 the half wall moves back and forth when opening the door to the room. Part of the wall is coming away which poses a risk for pinching of skin.
2. On the playground, broken Styrofoam was observed in multiple places. The plastic storage container was cracked with jagged edges.

In room #1, there was not a trashcan or disposal system that had a lid as required for soiled disposable diapers. Diapers were observed in the trash can.
