Inspector Notes
The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on Monday, September 18, 2023 from approximately 9:30 AM to 12:35 PM. There were 79 children in attendance and a total of four 12 supervising. A total of seven children and six staff records were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

5Evidence:
The records for Child #4 (first day of attendance 08/22/2023) and Child #7 (first day of attendance 08/22/2023) did not contain the name, address, and telephone number of at least one person designated by the parent to contact in case of an emergency and the parent cannot be reached; names of persons other than the custodial parent who are authorized to pick up the child; written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection for the provision of medical treatment on religious or other grounds, and a written statement that the vendor will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the vendor.

Cleaning materials were stored on a shelf in the children?s bathroom located in the Pre-K classroom.
Evidence:
A staff member in the Two?s classroom was observed changing a child?s diaper. After the diaper change, the child did not wash their hands. The child began participating in activities in the classroom.
