Inspector Notes
The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on September 4, 2024, from approximately 10:02AM to 1:17PM. There were 87 children in attendance and a total of 14 staff supervising. A total of 9 children and 9 staff records were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/12/2024. A POC submitted after this date will not appear on the public website.
Violations

1. The record for staff #6 (DOH:8/23/2021) contained a CPR certification that expired on 9/7/2023.
2. Staff #9 (DOH: 7/14/2023) did not have documentation of a current CPR certification.
3. Staff acknowledged the CPR certification being expired for staff #6 and confirmed staff #9 did not have any CPR certification. They stated they were unaware of the subsidy requirement of every staff maintaining a current CPR certification.
Evidence:
1. The record for staff #6 (DOH: 8/23/2024) contained a first aid certification that expired on 9/7/2023.
2. Saff #9 (DOH: 7/14/2023) did not have documentation of a current first aid certification.
3. Staff acknowledged the first aid certification being expired for staff #6 and confirmed staff #9 did not have any first aid certification. They stated they were unaware of the subsidy requirement of every staff maintaining a current first aid certification.
Evidence:
1. The record for staff #2, #6, #7, #8, and #9 did not contain documentation of the department?s annual health and safety update course.
2. Staff acknowledged the annual health and safety update courses had not been completed and stated they were unaware of the subsidy requirement to complete such training annually.
Evidence:
1. During the inspection, there was no working, battery-operated radio.
2. Staff acknowledged the radio previously on-site was broken last year and a new radio had not been purchased.
