Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A renewal inspection was initiated on November 4, 2021 and concluded on November 8, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were eleven children present, ranging in ages from two years of age to four years of age, for a total of 22 points with three caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of four child records and three caregiver records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-111-100-A
Based on review of children records, the provider failed to obtain documentation of a physical exam within 30 days of first day of attendance.

Evidence:
1. There was no documentation of a physical exam on file for Child #4. The first day of attendance for Child #4 was 09/07/2021.
3. The provider verified that there was no documentation of a physical exam on file for Child #4, and the first date of attendance for Child #4.
Plan of Correction: The provider will contact the parent and request the physical exam.
Standard 22VAC40-111-90-A
Based on review of children records, the center failed to ensure that immunizations were on file before the first day of attendance.

Evidence:
1. There were no immunizations on file for Child #4. Child #4's first day of attendance was 09/07/2021.
2. The provider verified that there was no documentation of immunizations on file, and the first day of attendance for Child #4.
Plan of Correction: The provider will follow-up with the parent and obtain the immunizations. In the future children will need to have immunizations on file before the first day of attendance.
