Inspector Notes

A monitoring inspection was initiated on 6/12/2023 at approximately 9:40am and concluded on 6/12/2023 at approximately 10:40am. There were a total of 8 children (20 points) observed with 2 caregivers present. The children were engaged in the following: toy play, circle games, singing, breakfast, bathroom breaks, hand washing, and other organized activities. The inspector reviewed compliance in the areas of administration, personnel and household members, physical environment, care of children, handwashing, diapering, medication, emergencies and nutrition. A total of 5 child records and 2 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of children's records, the provider did not have the name, address, and telephone number of each custodial parent's place of employment.

Evidence: There were no phone numbers of either parent's place of employment in Child #5's record.
Plan of Correction: Provider will obtain the work phone number for both parents.
Standard 8VAC20-800-830-A
Based on discussion with Provider, the emergency evacuation procedures were not practiced monthly with all caregivers and children in care during all shifts that children are in care.

Evidence: The most recent record of an emergency evacuation drill was 10/4/2022.
Plan of Correction: The Provider will document monthly emergency evacuation drills beginning 6/12/2023.
