During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 10/16/2025. The on-site inspection began at 12:35pm and ended at 2:25pm. The inspector reviewed compliance in the areas listed above. There were 9 children present and 3 caregiver;s the point total was 22.
The inspector reviewed 5 children?s records and 4 caregiver/household members records on-site on 10/16/2025. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with
applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
7Staff #2 was missing documentation of an emergency contact as well as documentation that two or more references as to character and reputation as well as competency were checked before employment.
Caregiver #2 had a TB screening on file that expired in October 2023.
Child #2 and Child #3 were missing an annual update of emergency information.
There was no leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal.
The portable battery-operated weather band radio was not in working condition.
There was no documentation of practicing monthly evacuation procedures since February of 2025.
Caregiver #1 was observed heating a bottle in a microwave during the inspection.
