Inspector Notes

An unannounced monitoring inspection was conducted today from 11:00am-12:50pm. There were 88 children directly supervised by 14 staff. The physical plant, 8 staff records, 10 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, singing, playing outside, and doing art projects. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard 22VAC40-185-500-A
Based on observation, one staff member did not wash their hands at required times.
Evidence:
In the Early Preschool room a staff member was observed to change three children's diapers without washing their hands with soap and water between each child.
Plan of Correction: The staff member will be retrained immediately and the topic will be reviewed with all staff at our upcoming staff meeting.

Based on observation, insect repellant was not kept in an area inaccessible to children.
Evidence:
A container of insect repellant was observed on a shelf in the Explorers classroom within reach of children.
Plan of Correction: Corrected during inspection.
Standard 22VAC40-185-60-A
Based on record review, one of ten children's records reviewed did not contain all required information.
Evidence:
The record for child 7 did not contain the name address and phone number for one of the two required emergency contacts.
Plan of Correction: We will contact the child's parents to get the information.
Standard 8VAC20-780-350-B-3
Based on observation, the ratio of one staff member for every eight two year old children was not followed.
Evidence:
Eight children and one staff member were observed in the Early Preschool room. For approximately two minutes a child from another class was allowed in from the playground to use the restroom, leaving the one staff member supervising nine children.
Plan of Correction: The staff member will be retrained immediately and the topic will be reviewed with all staff at our upcoming staff meeting.
