Inspector Notes
An unannounced monitoring inspection was conducted today from 11:00am-12:50pm. There were 88 children directly supervised by 14 staff. The physical plant, 8 staff records, 10 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, singing, playing outside, and doing art projects. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

4Evidence:
In the Early Preschool room a staff member was observed to change three children's diapers without washing their hands with soap and water between each child.
Evidence:
A container of insect repellant was observed on a shelf in the Explorers classroom within reach of children.
Evidence:
The record for child 7 did not contain the name address and phone number for one of the two required emergency contacts.
Evidence:
Eight children and one staff member were observed in the Early Preschool room. For approximately two minutes a child from another class was allowed in from the playground to use the restroom, leaving the one staff member supervising nine children.
