During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated and completed on February 12, 2025 in response to a complaint received by the licensing office on February 12, 2025 relating to allegations of classroom conditions and care of the children. This inspection included:
? tour of the facility,
? interviews,
? observations, and
? measurements (air temperature.
The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on March 5, 2025. A POC submitted after this date will not appear on the public website.
Violations
2Staff had been instructed by management to close the windows and door once the children were inside the classroom. Regardless, we retrained all staff on walkie code calls and the importance of communication and following direct orders. We reiterated staff can come to management if they are uncertain about information given to them.
After the painting of a classroom overnight, staff stated that they were instructed to move
the children into the classroom where the windows and door remained open due to paint odor. Children were observed lying on cots wearing coats and blankets and the air temperature of the classroom at cot level registered 53 degrees. Staff and management failed to ensure that the classroom environment was safe for the children to remain in care..

Staff had been instructed by management to close the windows and door once the children were inside the classroom. Regardless, we devised an improvement plan for all staff on the importance of communication on all things pertaining to the children and the school.
As the HVAC reading in the school stated 68 degrees, we had our HVAC company come out and check the system, which they found to be in good working order. To err on the side of caution, they are going to replace the thermostat of the classroom in question, as the system did not register the room being cold.
