During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 03/27/2025, as a part of the licensure period. The on-site inspection began at 10:00 AM and ended at 12:30 PM. The inspector reviewed compliance in the areas listed above. There were 136 children present and 25 staff. The inspector reviewed 8 children?s records and 8 staff records on-site.
This inspection included:

? tour of the facility,
? interviews, and
? observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on April 3, 2025. A POC submitted after this date will not appear on the public website.

Repeat violation
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
There was a container of hand sanitizer with a warning label on top of a cabinet in a classroom.
Plan of Correction: The facility responded with the following:
Reminded all staff that hazardous materials such as hand sanitizer must be locked in cabinets away from children.

Repeat violation
Staff shall wash their hands before and after a diaper change.
A staff person did not wash their hands following changing a child's soiled diaper.
Plan of Correction: The facility responded with the following:
Retrained and reminded all staff on when handwashing should occur throughout the school day.

Repeat violation
Disposable diapers shall be disposed in a leakproof or plastic lined storage system that is either foot-operated or used in such a way that neither the staff member's hand for the soiled diaper touches an exterior surface of the storage system during disposal.
When disposing a soiled diaper, a staff member placed the diaper in a plastic bag and put the bag in an open bin on top of the foot operated disposal system.
Plan of Correction: The facility responded with the following:
Retrained all diapering classrooms on how to use their diaper disposal system. We also purchased a new diaper disposal system that does not have an insert that will can be removed.l

The center shall maintain a written record of children's injuries that includes the how the parent was notified of the child's injury and the time the parent was notified of the child's injury.
An injury record did not have indicated the how the parent was notified of the child's injury and the time the parent was notified.
Plan of Correction: The facility responded with the following:
Retrained all staff on how to completely fill out accident reports to show date and time of parental notification. We also updated our current accident forms to reflect better spacing for date and time of parent notification.
