During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 07/31/2025, as a part of the licensure period. The on-site inspection began at 10:10 AM and ended at 11:10 AM, continued at 2:10 PM and ended at 3:45 PM. The inspector reviewed compliance in the areas listed above. There were 122 children present and 23 staff. The inspector reviewed 8 children?s records and 8 staff records on-site.
This inspection included:

? observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on August 8, 2025. A POC submitted after this date will not appear on the public website.
Violations
3Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
The cabinet above the children's toilet in room 10 was not locked. Numerous cleaning fluids and sanitizing agents with warning labels were stored in this cabinet.
We will retrain all staff on the importance locking all cabinets regardless of what is inside but especially the ones with chemicals. The leadership team will monitor this with weekly walkthroughs of the classrooms at random parts of the day to make sure all chemicals are locked away and all cabinets are locked.
Children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks.
Toddlers were observed getting up from their cots after nap time after going directly to the tables to eat afternoon snack without washing their hands.
We will retrain all staff of the importance of handwashing and when it should be occurring in the classroom. The leadership team will monitor this by doing observations in the classroom when handwashing should be occurring.
There were 5 prepared infant formula bottles in room 2 that were not dated. In addition, there were 3 prepared bottles with incorrect dates.
There were 6 prepared infant formula bottles in room 6 that were not dated. In addition, there were 2 bottles with incorrect dates.
We will retrain all infant staff on the importance of making sure all prepared bottles and food have the correct dates and labels when being brought into the classroom. The leadership team will monitor this by randomly checking infant refrigerators to make sure all bottles and food is labeled correctly.
