Inspector Notes

An unannounced complaint inspection was conducted on 3/22/2024 from 11:50am to 1:30pm, in response to a complaint that was received by the department regarding forbidden actions and behavioral guidance. Staff interviews and observation of the classroom related to the complaint were made. The preponderance of evidence does support a valid finding regarding standards related to behavioral guidance and forbidden actions. Areas of non-compliance are identified on the Violation Notice. Complaint Related

Based on interviews with lead classroom teacher and center director, it was determined behavior guidance was not constructive in nature and was not intended to redirect children to appropriate behavior and resolve conflicts.

Evidence: When a child was exhibiting unsafe behavior, Staff #1 flicked water with their fingers at the child in an attempt to calm them down.
Plan of Correction: We've offered staff positive guidance feedback and additional training to enhance skills in challenging situations with children positively.

Complaint Related

Based on interviews with lead classroom teacher and center director, it was determined that the forbidden action of enclosing a child in a confined space that the child cannot freely exit himself happened on March 19th 2024.

Evidence: Staff #1 and Staff #2 confirmed that a child was taken into the classroom bathroom with the teacher blocking the open door when this child was exhibiting unsafe behavior.
Plan of Correction: We developed a behavior support plan tailored to the child's needs, including prevention strategies, de-escalation techniques and safety protocols. We continue to monitor & adjust the plan to ensure the safety of the children.
