Violations

1
Standard 8VAC20-780-270-A
Based on observation, areas and equipment of the center, inside and out, were not maintained in a safe and operable condition. Unsafe conditions included visible cracks, breakage of equipment, chipped paint and head entrapment hazards.

Evidence: There was a cracked water table, fence with post not attached to the chain link, torn drywall in the hallway, chipped paint in a classroom, and netting that created a head entrapment hazard on the playground.
Plan of Correction: Center will replace the water table, repair the fence and drywall, repaint the classroom, and remove the netting that caused the head entrapment hazard.
