During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 4/21/2025. The on-site inspection began at 11:00am and ended at 12:55pm. The inspector reviewed compliance in the areas listed above. There were 27 children present and six staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-350-B-4
The required ratio for children ages three years to school ages eligible is one staff per ten children. Staff #1 was observed supervising 11 children (youngest child three years old) from approximately 11:10am-12:30pm.
Plan of Correction: A substitute staff arrived at 12:30pm to correct the ratio. The center will ensure that ratios are met even when a staff member is unexpectedly absent.
Standard 8VAC20-780-420-E-3
Staff shall request at least annually parent confirmation that the required information in the child's record is up to date. Up-to-date information is required to be requested from the parent for each child's record at least annually. The information in the record of child #2, #3, #4, and #5 had not been updated in over three years. Which is overdue by two years.
Plan of Correction: The center will request that parents verify their information is up to date and document the date. The center will ensure that parent's confirm their information is up to date annually.
