Inspector Notes

A monitoring inspection was conducted on 4/25/24 from 11:55 am to 1:15pm. There were 28 children present, with 5 staff directly supervising. A total of 3 children's records, 3 staff records, attendance, emergency drill records, medications and other center documents were reviewed. The children were observed arriving from the playground, using the bathroom, eating lunch from home and taking naps.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. For any questions regarding this inspection, please contact me at mahrukh.aziz@doe.virginia.gov or 571-835-4718.

Standard 22.1-289.035-B-4
Based on records review, the center did not obtain the results of an out of state criminal record check (CRC) for an employee who resided outside of Virginia within the last 5 years, prior to the first day of employment; and the center did not obtain an employee's out of state central registry search (CRS) within 45 days of employment.
Evidence:
1. Staff # 1's first day of employment was 3/14/24. Staff # 1's record indicated that Staff # 1 resided in Washington D.C within the last five years.
2. A CRC request had not submitted for Staff # 1.
3. Staff # 2's first day of employment was 7/5/21. Staff # 2's record indicated that Staff # 2 had resided in Maryland within the last 5 years.
4. Staff # 3 stated that the Maryland CRS was submitted for Staff # 2 during the summer of 2021 and the results never came back. The search was resubmitted on 10/10/23 with the same outcome. The CRS was submitted for the third time on 4/10/24.
5. There was no documentation to show if a follow up was completed to check on the status of these applications.
Plan of Correction: The CRC will be submitted and the licensing inspector will be informed when the results have been obtained. The CRS has been submitted on 4/10/24.
Standard 8VAC20-780-560-F-4
Based on observation, children three years and younger were eating foods that are considered to be potential choking hazards.
Evidence:
1. Child # 1 (2 years old) was observed eating a hot dog that was cut horizontally in whole pieces.
2. Child # 2 (2 years old) was observed eating grapes that were cut horizontally in two halves for each grape.
Plan of Correction: Parents will be reminded to cut their child's lunch according the regulations and staff will check lunches to ensure that all food items are of appropriate size, so they do not pose a chocking hazard.
