Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was conducted on 5/14/2021. The provider was contacted by telephone to initiate the inspection. There were 5 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records, 2 staff records, and 4 household member records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on record review, the provider did not obtain an updated tuberculosis (TB) screening or test for all staff and household members completed at least every 2 years.

Evidence: Staff #1's last TB test was completed 5/8/2018. Adult Household Member #1's last TB test was completed 12/12/2018. Adult Household Member #2's last TB test was completed 5/8/2018.
Plan of Correction: He's busy taking his finals. As soon as he will be done I will take him for TB testing.

Based on record review, the provider did not ensure that all adult household members have a completed Sworn Disclosure Statement on file.

Evidence: Adult Household Member #3 does not have a Sworn Disclosure Statement on file.

Based on record review, the provider did not ensure that all adult household members submit to a fingerprint based criminal history background check.
